Data note: This is not a comprehensive list of home visiting providers in the state; rather, it is those known to be providing home visiting through the major federally and state funded contracts as of 2024.

Agencies are categorized by program type. Maternal and Child Health / Healthy Families America is federally funded; Comprehensive Family Support Services are contracts under New Hampshire Department of Health and Human Services, Division of Children Youth and Families (an effort by several state agencies to blend federal funds to provide home visiting); Birth to 3: Family Centered Early Supports and Services is federally funded by Part C of IDEA (services for children with developmental disabilities or delays, 0-3). Agencies that operate programs under more than one of these funding sources are also indicated.
